      ILD 및 폐질환의 원인 규명을 위하여 조직학적 검사를 하는 경우는 꽤 드물다. 그래서 외워지지도 않는다. 다음 주 수요일에 conference 가 있으니 그 때까지 족보를 좀 봐두어야 겠다.

      정말 가끔씩 볼 수 있는 것이다. 신장 이식 환자들에게서 antibody-mediated rejection 이 있을 때 C4d의 침착이 관찰된다고 한다. 
     
      아래쪽 사진은 ABO mismatch 신장 이식 증례이다. 임상 요청이 들어와서 원래 예정에 없는 IgM 에 대한 염색을 시행하였는데 C4d가 침착되는 곳과 같은 곳인 peritubular capillary 에 IgM 의 침착이 관찰되었다. 요청하는 장기이식 센터의 선생님의 말씀에 의하면 침착이 관찰될 수 있지만, 임상적인 의미는 부여할 수 없는 그런 상황이라고 한다.

      ABO mismatch 인 신장 이식이 증가하면서 acute antibody-mediated rejection 이 자주 관찰되는데, 병리 소견으로는 rejection 이 맞는데 임상적으로는 no rejection 인 경우가 많아서 서로서로 피곤하다. ㅡㅡ

      참.. 오늘의 교훈은 병리과에 요청을 할 일이 있으면, 명확하게 표현을 해야한다는 것! IgM 염색도 요청하지 않았으면, 그냥 지나갈 뻔 한 소견이다.

  • IgA nephropathy

     IgA nephropathy is a glomerular disease in which glomerular
    immunohistologic staining for IgA is more intense or equally intense as
    staining for IgG and IgM.

      IgA 신증은 꽤 흔하게 볼 수 있는 질환이기 때문에 정의를 꼭 알아야 하는 질환이다. 딸랑 저 한줄 뿐만이 아니라 다른 배제 기준이 있기는 하지만 기본이 가장 중요하고 길면 잘 안외워지니깐.. 🙂
